Change of service provider manager Form R5

The Change of service provider/manager (Form R5) is used to notify the resident/s and the RTA that there has been a change of service provider/manager of the rental premises while a rooming accommodation agreement is continuing. This helps with the rental bond refund process.

It must be completed by the previous service provider/manager and new service provider/manager.
