The Provider/agent signature record (Form R8) is used to notify the RTA that there have been additions or deletions to the authorised signature list of provider/agent's personnel. It records the names and signatures of persons who are authorised to sign RTA forms about rental bonds. Failure to provide current signatories can result in delays with rental bond refunds.

The form is completed and signed by the provider/agent who authorises each signatory and signed by each new person.

The next step is to send it to the RTA immediately when there is a change in authorised personnel.
