Change of Lessor or Lessor's Agent - Form 5

Purpose of this form

To notify the tenant/s and the RTA that there has been a change of lessor/agent of the rental premises while a tenancy is continuing.

Form completed by

Previous lessor/agent and new lessor/agent completes the form.

The next step

The new lessor/agent sends a copy of this form to the RTA and to the tenant/s immediately there is a change.

If you receive this form as a tenant

You should make future payments of rent to the new lessor/agent or as directed by the new lessor/agent.
