Form R1- Condition Report

Purpose of form

If a rental bond is required or has been paid, the service provider/manager must fill in this form to record the details of the condition of the resident's room at the start of the residential service agreement.

Who signs the form

This form must be signed by both parties and a copy given to the resident.

The Condition Report can be used as evidence of the condition of the room.
