Purpose of this form

This form is used during the tenancy where either the tenants or the lessor/agent claim that there has been a 'breach' of one (or a number) of terms of the residential tenancy agreement.

Form completed by

Either the tenants or the lessor/agent to the party who is 'in breach'.

If you receive this form

It means that the other party is letting you know there is a problem or dispute, and you need to take action to rectify the situation within a certain time.
