Purpose of this form

To notify the RTA that there have been additions or deletions to the signature list of property management personnel. It records the names and signatures of persons who are authorised to sign RTA forms.

Form completed by

The principal/owner completes the form and authorises each signatory and signed by each new person.

The next step

Send it to the RTA immediately after any changes to authorised personnel.
